Sports Physicals

What is a Sports Physical?

A sports physical is a health and fitness assessment where, in addition to reviewing a student's medical history, special attention is paid to look for any indications of diseases or injuries that would make it unsafe for a student to participate in a specific sport.  

When Can I Get One?

At your convenience!  We recognize that you have a busy schedule and that these exams, in addition to all of the other preparation work that you do for your student(s), can sneak up on you.  Therefore, walk-ins are welcome!  

What Do I Need to Bring?

Please note that you will need to supply the school's form(s) for the student's participation in sports for completion by Dr. Digney at the time of the examination.  

These exams do require a detailed health history and signed consent form which we encourage you to have ready before your visit.  For your convenience, these forms are available for you to download/print via the link below.  

All individuals under the age of 18 will need a parent/guardian's signature on the "Sport Physical History-PPE" and "Informed Consent" forms prior to the examination.
